Enrollment History Records

Student Records is currently closed to walk-ins due to the Coronavirus. For this reason, access to records is currently limited. 

Enrollment History records can be obtained from the Student Records Department. The Student Records Department requires the following in order to release your enrollment history records to you:

  • a valid photo ID such as a license or passport;
  • a phone number where you can be reached;
  • the name that you were registered under when you were a student;
  • your birthdate;
  • the name of the school that you last attended in our district; and
  • your actual signature-not electronic.

Your student ID number is helpful in finding your record, but not required. 
You can fax this information to 909.473.0052. At this time, all requests will only be processed to an email address.
