Enrollment Forms

Enrollment and Placement Services is open year round except major holidays. We are open for enrollments Monday through Friday 8:00 a.m to 4:00 p.m.
Enrollment and Placement Services enrolls students for elementary school only. Families have the option to enroll a child at Enrollment and Placement, or the school of residence. Middle school and high school enrollments must be done at school of residence. For more questions, please call our department at (909) 889-7576.
Print Student Registration Forms below. Carefully complete and sign all forms. Student Registrations forms must be turned into Enrollment and Placement Services or school of residence along with:
          - Birth Certificate
          - Immunization Record
          - Proof of Address 
          - Parent/ Guardian's Identification
Students will be enrolled to school designated to the address he or she resides in.
